Appointments
Patients are seen by appointment in both the Southampton and Riverhead offices. We make every effort to stay on schedule. If, due to unforeseen events, we are behind schedule, we will try to notify you so that you can decide on how to best use your time. Please allow adequate travel time so that you can be seen promptly at the appointed time.

Cancellation
Time is reserved exclusively for you. If you are unable to keep an appointment please notify us as early as possible so that another patient may be helped.

Fees
The fees in our office are based on the care, skill and judgement needed to best address your needs. We can discuss the fees for your care prior to your treatment.

Referrals
There is no higher compliment you can pay us than to entrust us to care for your friends and family. We are always pleased to accept new patients.

Financial
Our financial policy is payment of fees on the day of service. For your convenience, we accept cash, check, Visa, MasterCard, and American Express cards. Payment options and plans are available for patients with advanced treatment needs.

Insurance
Please bring the necessary completed forms on the day of your appointment. Our staff will handle filling out forms for both pre-determination of benefits as well as reimbursement to you by your insurance carrier.
We will be happy to provide any necessary documentation.

Medical Alerts
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require antibiotic premedication prior to dental visits.
